Sage Physical Therapy and Wellness is located in a private clinic suite within a newly renovated professional office space in the Benbrook-area of Southwest Fort Worth. Doors will remain locked during appointments for patient privacy and walk-ins are not available.
In addition to on-site visits, Sage also offers in-home and virtual services for those who prefer the convenience of care outside the clinic. Please note that travel fees may apply for homes located more than 10 miles from the clinic.

In short, no!
In Texas, as of 9/1/2025, you can see a Doctor of Physical Therapy for 30 days prior to requiring a referral (Legislation House Bill 4099).
If your treatment exceeds 30 days, we will require a referral, which is generally easily obtained from a licensed healthcare provider (MD, DO, PA, NP), sometimes as easily as a phone call!
If you would like to avoid a lapse in care, having a referral at the start of treatment can be helpful, but is not required. Please provide referral-in-hand or have referral faxed to 682-255-0030.

Sage Physical Therapy & Wellness is a cash-based practice built on the belief that patients deserve focused, personalized care. With insurance reimbursement cuts and increasing limitations, many clinics are forced to see multiple patients at once just to stay afloat. Here, you’ll always receive private, one-on-one treatment designed to address your unique needs and goals.
For those who wish to use insurance, Sage can provide a superbill upon request. This can be submitted to your insurance company for possible or partial reimbursement; however, reimbursement is not guaranteed and is the patient’s responsibility.
MEDICARE: We are currently in the process of credentialing with Medicare and would love for you to reach out so that we can keep you informed on this process.

Everyone’s recovery looks a little different. Some people notice improvement in just a few visits, while others benefit from a longer course of care. After your initial assessment, your therapist (Sarah) will discuss her recommendations based on your goals and needs.
With that being said, the decision is always collaborative and your plan can be adjusted at any time to make sure it feels like the right fit for you.
